Apply for an annual blanket utility permit

A blanket utility permit allows you to perform normal maintenance and repair work to existing facilities and service lines throughout the City of Effingham for a year. This includes:

  • Normal maintenance and repair
  • Relocation and replacement (in the same location)
  • New service lines
  • Restoration of lawn, sidewalk, and driveway surfaces

Before you start

Review all the terms and requirements in the annual blanket utility permit application.

If the length of the relocation or replacement is over 1,000 feet or there are road crossings, you will need to submit plans with your application.

What to do

  1. Download and complete the forms within the annual blanket utility permit application.
  2. Gather proof of insurance with the City of Effingham named as the certificate holder.
  3. Download and complete the bonding forms within the annual blanket utility permit application.
  4. Submit your application and the required materials by mail or in-person at the Public Works office in City Hall.
    • Mail: City of Effingham, attention Public Works, P.O. Box 648, Effingham, IL 62401
    • In-person: Public Works, 201 East Jefferson Avenue
  5. Pay $1,500 permit fee.
